Urologic problems which affect females require a special approach to diagnosis and management. Urinary incontinence a common and often debilitating problem is a common problem treated by the urologists at the University of Kansas Hospital. There are several different types of incontinence, each requiring a unique approach to treatment. Dr. Tomas Griebling, the only fellowship trained female and geriatric urologist in Kansas City, offers the latest techniques in the diagnosis and management of difficult female urologic problems. Furthermore, with ongoing research, Dr. Griebling continues to search for more effective ways to manage complicated urologic problems in females and the elderly.
Incontinence occurs in many forms. For example there is "stress incontinence" which usually occurs with increases in abdominal pressure such as coughing, sneezing, or laughing. Another type of incontinence is "urge incontinence" which is the inability to hold the urine once the urge occurs. Many times the type of incontinence is not straightforward and may require special diagnostic testing. In the Department of Urology at the University of Kansas Hospital, we offer the latest equipment used in diagnosing incontinence and other problems of the bladder. Our Video Urodynamic Facility is “state of the art” equipment to provide specific information about even the most complicated conditions of the bladder. In addition, Dr. Griebling has specialized training in interpreting the results of such testing ensuring the patient of the proper management of their condition.
Our specialists are experienced in all forms of effective treatment for incontinence and other female urologic conditions ranging from biofeedback and medication to surgery. Dr. Griebling is experienced in the treatment of both simple and complex urinary problems including patients who have failed previous treatments elsewhere. Our surgeons use a wide variety of cutting-edge techniques such as urodynamics, biofeedback, periurethral injection therapy, nerve stimulator implants, and innovative drug combinations for evaluation and management of incontinence and interstitial cystitis syndrome. If surgical reconstruction is necessary our surgeons specialize in the repair of lower urinary tract abnormalities such as genitourinary fistulae, urethral diverticula and complex pelvic prolapse. In addition, our physicians employ a wide variety of procedures for stress urinary incontinence including sling procedures.
Dr. Griebling has also been a pioneer in the area of implants for sacral nerve stimulation which provides effective treatment to many of those patients who have failed all other forms of therapy for incontinence, pelvic pain, and bladder dysfunction. Interstitial cystitis, an inflammatory condition of the bladder, is often a disabling condition, but in some cases may be treated with sacral nerve stimulation.
We also specialize in male urinary incontience and artificial urinary sphincters and can be evaluated by our specialist Dr. Broghammer.
SATISFYING SOLUTIONS Effective Treatments for Male Incontinence
Many men suffer from bladder control problems, often as a result of diabetes or prostate surgery. Male urinary incontinence is usually caused by a damaged sphincter, the circular muscle that controls the flow of urine out of the bladder. When damaged, often the unavoidable result of prostate cancer surgery, the muscle cannot squeeze and close off the urethra. This causes urine to leak.
Incontinence can limit your activities… keep you homebound… in costly, messy pads…dealing with the frustration and embarrassment, and the constant fear of leaking and odor. The reality is, you don’t have to live with bladder control problems anymore.
Our practice specializes in real solutions… from minimally-invasive surgical procedures like the male sling for mild to moderate incontinence, to the gold standard artificial urinary sphincter implant that can resolve even severe incontinence.
MILD TO MODERATE INCONTINENCE
AdVance® Male Sling System
• Small sling made of synthetic mesh placed inside the body through small incisions
• Supports the urethra, restoring normal bladder control
• Most patients are continent immediately following the procedure
• Can resume normal, non-strenuous activities shortly thereafter
MILD TO MODERATE INCONTINENCE
InVance® Male Sling System
• Also involves placing a mesh sling completely inside the body
• Places pressure on the urethra to reduce the possibility of urine leakage
• Most patients are continent immediately following the procedure
• Can resume normal, non-strenuous activities shortly thereafter
MODERATE TO SEVERE INCONTINENCE
AMS 800® Urinary Control System
• Artificial urinary sphincter placed completely inside the body
• Provides simple, discreet urinary control
• Mimics a healthy sphincter, keeping the urethra closed until you want to urinate
• • Connected system of:
• o Pump implanted in the scrotum
• o Inflatable cuff around the urethra
• o Balloon reservoir implanted in the abdomen
HOW IT WORKS
1. Control urination by squeezing and releasing the pump
2. Moves fluid out of the cuff and back into the reservoir
3. Urine can flow out of the bladder
4. Fluid returns from reservoir to cuff, squeezing urethra closed again
